Application
New Students:
Thank you for your interest in OLMC, we are delighted that you are considering applying to our school.
To begin the application process, please complete the online registration through FACTS.

A $25 application fee is required and must be paid through the FACTS portal.
The following documents must be uploaded to FACTS before registration can be completed:
- Birth Certificate
- Baptismal Certificate
- Child’s Immunization Record
- Church Envelope Number (required to obtain the Parishioner Tuition Rate)
- Most recent Report Card
- New York State English Language Arts (ELA) and Mathematics Exam results (if applicable)
Pre-K 3 Eligibility:
- Children entering Pre-K 3 in September must be 3 years old by December 31, of that same year. (Must be potty trained)
Pre-K 4 Eligibility:
- Children entering Pre-K 4 in September must be 4 years old by December 31, of that same year. (Must be potty trained)
Kindergarten Eligibility:
- Children entering Kindergarten in September must be 5 years old by December 31, of that same year.
Returning Students:
Welcome back! We are excited to continue partnering with our returning families!

All fees are paid through the FACTS Family Portal:
- Application Fee: $25.00 (for each new registrant only)
- Registration / Re-Registration Fee: $125.00 per child
- Parents’ Guild Fee: $100.00 per family (includes one Parents’ Guild “Lucky 200” ticket)
